   address, telephone number, email address, Medicare number (where applicable), DVA details,
   health fund details, emergency contact information, clinical records relating to dental and oral
   health, relevant medical history, referral information and payment information.

   We collect information when you contact the clinic, book appointments, complete forms, receive
   treatment, or communicate with us. We may also receive information from dentists, medical
   practitioners, specialists, hospitals, health funds and government agencies where authorised by
   law.

   appointments and treatment, process payments and claims, meet legal obligations, respond to
   enquiries and improve services.

   health funds, Medicare, DVA, professional advisers and government authorities where required by
   law. We do not sell personal information.
